Patient Instructions for Accessing Medical Records and Filing Complaints
In accordance with Texas Health & Safety Code Sections 181.103 and 181.105, we are providing the following instructions to help patients:
- Request copies of medical records
- Contact the appropriate licensing or disciplinary authority
- File a consumer complaint regarding privacy or healthcare concerns
How to Request Your Medical Records
Step 1: Submit a Written Request
You may request a copy of your medical records by submitting a written request to our office.
Requests may be submitted by:
- In person through our office
Please include:
- Your full name
- Date of birth
- Phone number or email address
- Information to be released
- Dates of service, if known
- Your signature
Step 2: Provide Identification
For your privacy and security, we may require a valid government-issued photo ID before releasing records.
If someone is requesting records on your behalf, additional authorization documentation may be required.
Step 3: Processing Time
We will process your request in accordance with applicable Texas and federal law.
You will be notified if:
- Additional information is needed
- Fees apply for copies
- Records are unavailable
Step 4: Receive Your Records
Records may be provided:
- Electronically
- By in-person pickup, if available

How to File a Privacy or Consumer Complaint
You have the right to file a complaint if you believe your privacy rights have been violated or if you have concerns regarding healthcare services.
You may file a complaint directly with our office and/or with the appropriate state or federal agency.
Step 1: Contact Our Office
Please submit complaints to Dr. Todd to see if he can remedy the situation.
You will not be retaliated against for filing a complaint.
Step 2: File a Complaint with the Texas Attorney General
You may file a consumer complaint regarding medical privacy concerns with the Texas Attorney General.
